Support New Jersey’s goal of reducing, and eventually eliminating, health disparities in the state.

Overview and Objectives

Partnered with the Cumberland County Health Department for the 2013 Chronic Disease Self-Management Program. As part of this program, provided a six-week Tomando Control de su Salud (Taking Control of Your Health) workshop for Spanish speaking adults age 60 and older.
